A violent incident at Oudtshoorn Correctional Centre on Wednesday, 7 August 2025, left an inmate dead and two wardens seriously injured. The Oudtshoorn inmate killed had been in the Medium A section of the facility when he attacked the officials during an escort.
According to the Department of Correctional Services (DCS), the inmate used a sharpened object to stab the wardens. Both sustained serious injuries before nearby officers restrained the attacker.
Wardens Respond and Contain the Threat
Responding officials quickly intervened and restrained the inmate, using what the department described as “minimum force.” After the altercation, the inmate collapsed. Paramedics transported him to a hospital, where doctors confirmed his death.
The DCS confirmed the injured wardens are stable and receiving treatment. The identities of those involved have not yet been released.
Mental Health Concerns Surface
The Oudtshoorn inmate killed had previously undergone a psychological assessment and was receiving treatment at the time of the incident. Officials are now investigating whether mental health played a role in the violence.
An internal probe has been launched to determine the sequence of events and assess whether current safety protocols were followed.
Spotlight on Prison Safety
This deadly incident involving an Oudtshoorn inmate killed after attacking staff highlights deeper challenges in the prison system. Staff members often work in high-risk environments, especially when managing inmates with psychological needs.
Correctional Services is offering trauma counselling to affected personnel and reviewing safety measures at the facility.
The department said it will share more information once the investigation concludes. It also asked the public and media to avoid speculation during the process.
The fatal clash adds to growing pressure on South Africa’s correctional system to improve mental health care and strengthen officer protection within prisons.
